The first visit is to evaluate your condition and if needed order further investigations. Please allow time for your first visit (between 30-60min). When you come for your appointment please remember to obtain and bring the following:

• Referral letter from GP, family physician or other doctor
• List of medications
• Please bring copies of operation reports (if you have them)
• Medicare card, DVA card, pension card

Have your private hospital insurance information with you.

Please print and fill the following forms and bring it with you at your first visit We endeavour to run on time, but unexpected delays can occur rarely with unplanned arrival of babies. If such delays occur we will inform you. If you are travelling from long distances, your appointment will not be cancelled.
